Some more tests: also in environments works this, e.g.

\begin{description}
\item[]...
\end{description}

is transferred to

\begin{description\item{description}}
\item[]...
\end{description\item{description}}

Then, the document is not compileable anymore.

Unfortunately, the \index{} are also added in \label{} and \ref{}. This limits the usage of the great program because the labels must be adapted.
