I have exactly the same things when the weather is wet ....
I think that this is a mixture of bad weather + state machine

Some minutes after Modem Operationnal, I get
Feb 23 20:27:06 localhost kernel: [EAGLE-USB] SM: LOS Defect
Feb 23 20:27:06 localhost kernel: [eagle-usb] SNR margin < 0 . Rebooting
Feb 23 20:27:27 localhost kernel: [eagle-usb] Modem operational !!

This is a problem with the line (loss of signal). Some debug would be necessary to identify the conditions in which it happens (maybe add the attenuation, ... , what processes were running - dhcpcd in my case - ...)

The synchronization is ok just after so that's not a big problem...
A user had this regularly because of big winds (the wire was not buried for his telephone line) so check the weather forecast !
