Someone suggested it before (at CERN I think).

There should be, like in bugzilla, a way to check for duplicates reports (maybe just as option). When someone submit an item, that would do a search of items of the same project (maybe restricting the research on the category) and shows at least the first 20 items found. That would help avoiding duplicates.
